U of T IB conditional acceptance Options

Some say one must maintain his/her current standing, while others say one only needs to attain his/her IB diploma. (That is, completion of CAS, EE, TOK, and total 24 points in the IB subjects) Which one is ture?

Also, if one fails to satisfy the offer of admission of IB predicted results, will his/her high school transcript marks still be considered?
